I fully understand that my child’s participation in an Earth Team event or field trip (hereinafter "Event") involves inherent risks, including the risk of personal injury, loss of life or property damage. I hereby acknowledge that I am voluntarily allowing my child to participate in this Event and agree to assume any such risks.
I hereby release, discharge and agree not to sue Earth Team for any injury, loss of life or damage to or loss of personal property arising out of or in connection with my child’s participation in the Event from whatever cause, including the active or passive negligence of Earth Team or any other participants in the Event.
In consideration for being permitted to participate in the Event, I hereby agree, for my child, my heirs, administrators, executors and assigns, that I shall indemnify and hold harmless Earth Team from any and all claims, demands, actions or suits arising out of or in connection with my child’s participation in the Event.
No known physical or health limitation prevents my child from safely participating in this Event. My child will perform only those tasks assigned, observe all safety rules, and will use care in the performance of his or her assignments.
I give permission to authorize personnel to carry out such emergency diagnostic and therapeutic procedures as may be necessary for my child, and also permit such treatment procedures to be carried out at, and by the local hospital(s) for my child in the event of an emergency.
I understand that any medical expenses will be billed directly to me or my insurance company. I consent to the minor being driven by automobile, by a licensed, insured Earth Team staff member or volunteer, to or from the Event.
I agree to permit Earth Team staff, agents and other guests to take photographs and/or videotape during Earth Team events and field trips without further recourse. I understand that such photographic images, video, or audio recording of my child may be used for commercial and/or promotional purposes. I understand that personal information will be collected and kept on file for my child and agree to the release of the information for reporting purposes.
